How It Works

Step 1

Get to know you

As soon as you join Galileo, we’ll connect you to your personal care team–a dedicated lead medical provider and health advocate–that will support you throughout your Galileo experience, along with our team of experts in specialty medical care, mental health, and social work. Your care team works closely together to help you manage your health. They’ll start with a conversation about your needs and goals to build a trusting, long-lasting relationship with you.

Step 2

Meet your care team for a full check in

In- Home: Our care team (your lead primary care provider and health advocate) will come to you for your first visit. We ask that you invite any family members or caregivers so we can meet them as well. Here’s what you can expect: - We’ll ask about your medical history and specific health concerns in detail - We’ll then take vital signs like blood pressure, height and weight - We’ll review all of your medications with you - We’ll spend some time meeting the other members of your family and/or any caregivers present - Your first appointment is longer than a traditional doctor’s appointment to leave plenty of time for your providers to get to know the whole you Virtual: A virtual initial visit is another way for you to get to know your care team. We can cover many of the same concerns as an in-person visit, all fitting into your busy schedule. Here’s what you can expect: - You’ll see your primary care provider and health advocate over Zoom video technology using your phone or computer - We’ll review your medical history and specific health concerns in detail - We would love to meet the other members of your family and care team if they are available - We will review your medications together - If you have a scale, blood sugar meter or home blood pressure machine, we will review these numbers with you

Step 3

Seamless coordination across specialists when needed

With access to Galileo’s internal network of specialists covering any medical problems that arise, your provider may be able to treat you and develop a care plan without the need for additional doctors and visits. For all other cases, if you have existing specialists we’ll partner with them to manage your care. If needed, your care team can refer you to in-network specialists from our curated list of top specialists in the community. In both cases, we’ll make sure they have all your records and information prior to your visit, and follow up with you after your appointment to make sure all your questions are answered.

Step 4

Convenient and reliable support 24/7

As a Galileo patient you can call or text us anytime you have a health concern or question (even the middle of the night). If you need care, our doctors can treat you over the phone, make an appointment for you to see someone in person, or work hand-in-hand with urgent care or the ER if needed.
